Meal Preparation

Service Direct supports meal preparation in a way that promotes choice, dignity and independence. We help people prepare meals of their choice, based on their preferences, routines, cultural needs and dietary requirements. Where possible, we encourage people to stay involved in preparing food so they can maintain confidence, daily skills and control at home.

Personal Care

Service Direct provides respectful personal care that promotes independence, protects dignity, privacy and choice. We support people in line with their assessed needs, routines and preferences, encouraging independence wherever possible. Staff work at the person's pace, explain what they are doing, gain consent, and ensure care is delivered sensitively, safely and consistently.

Shopping

Service Direct supports shopping in a way that promotes independence, choice and community involvement. We can take people shopping or go shopping on their behalf, depending on their assessed needs and preferences. Staff can support with shopping lists, choosing items, carrying bags, unpacking and putting shopping away safely at home.

Medication Management

Service Direct provides safe medication management and support in line with each person's assessed needs, care plan and prescribed instructions. We promote choice, dignity and independence by supporting people to understand and follow their medication routines. We can also support with medication collection.

Transportation Services

Service Direct supports transport services to help people remain independent, connected and involved in their community. We can support people to attend appointments, access local services, visit family or take part in social activities. Support is planned around the person's choices, mobility needs and safety, promoting confidence, routine and control.

Safe and Well Visits

Service Direct provides Safe and Well Visits to offer reassurance, oversight and friendly support. These visits help confirm that a person is safe, comfortable and coping well at home. Staff can check general wellbeing, mood, hydration, home environment and any immediate concerns, while promoting dignity, independence and peace of mind.

Cleaning and Domestic Support

Service Direct supports cleaning and domestic tasks to help people maintain a safe, comfortable and well-kept home. Support is provided in line with the person's needs, preferences and routines, and may include light cleaning, laundry, changing bedding, washing dishes and general household tasks that promote dignity, independence and wellbeing.

Housing Support and Independent Living

Service Direct provides housing support to help people maintain safe, stable and independent living. We can support with tenancy-related tasks, budgeting, letters, appointments, household routines, accessing local services and building confidence to manage day-to-day responsibilities. Support is person-centred and focused on preventing tenancy breakdown, reducing isolation and helping people live as independently as possible.

Fall Prevention

Service Direct promotes falls prevention through early identification, regular assessment and responsive support planning. We consider mobility, home environment, equipment needs and changing risks, and where required, we refer for assistive technology, home adaptations and specialist equipment. Our aim is to reduce avoidable falls while supporting people to remain safe, confident and independent at home.

Days Out and Community Support

Service Direct supports people to enjoy meaningful days out and stay connected with their local community. This may include attending day centres, community centres, lunch clubs, libraries, wellbeing groups, craft sessions, local events, garden centres, museums, coastal walks, church groups or activities linked to the person's interests. Support is planned around each person's confidence, mobility, routine and safety.

Learning and Life Skills Support

Service Direct can support people to access education, training and employment opportunities where this forms part of their personal goals. This may include support to attend courses, volunteering, work placements, interviews, community learning or skills-based activities that build confidence, independence and future opportunities.

Companionship Support

Service Direct provides companionship support to help people feel connected, valued and less isolated. We can spend time with people at home or in the community, offering friendly conversation, reassurance and support with meaningful activities. This may include hobbies, games, walks, visits, appointments or simply having someone trusted to talk to.
